At Fresh River Healthcare, our Social Services department is staffed with concerned and dedicated professionals.
The team helps residents adjust to becoming members of their new community. They work with residents on a one-to-one basis and serve as a liaison between residents, staff and families. They are here to answer questions and offer support and guidance.
Among many other services, residents and families can expect a 48-72 hour meeting, care planning conferences and discharge planning for the nest site of care.
Our entire team specializes in residents who have complicated clinical and social needs and we are here to help bridge gaps and make vital connections to the community.
